Search for the contact you wish to make a "Note" for using the "Search Contacts" bar. Type in their name and click the contact as it shows up. This will take you to their specific contact record.

To create the "Note": Type in the note you wish make about the contact in the blank box. Click "Add Note". A message will appear above letting you know you have created it successfully and the "Note" will also appear at the bottom.

You can always "Edit" or "Delete" the "Note" by Clicking on the Pencil or Trashcan icons respectively.
